Full-time home care that delivers results

We work with discharge teams, local authorities and families across the country, to provide full-time live-in care that reduces delayed transfer occupancy and gets patients back home fast.

We understand the pressures being felt by professionals across the social care system. With budgetary constraints, rising demand and increasingly complex needs exacerbating capacity shortfalls, time is of the essence to increase discharge efficiency ahead of winter pressures.

Our discharge scheme is a fast, flexible and efficient service geared towards patients’ needs, offering both short and long-term care.

Why Elder for hospital discharge?

Guaranteed supply of care professionals

Our proprietary logistical and operational platform means we’re adept at responding to regional variations in supply and demand. It means we can guarantee care provision, anywhere in the UK, within 24 hours.

A solution proven to reduce readmission

We work with commissioners and healthcare professionals to gain a detailed understanding of a patient’s individual needs. Our model focuses on one-to-one, full-time support, with proven reductions in some of the primary causes of readmission, such as trips or falls.

Working in synergy with HCPs

Elder carers support outpatients in recovery and rehabilitation and are experienced in working closely with district nurses, social support workers and general practitioners.

Adaptable care for acute and non-acute needs

With care experiences spanning simple companionship to advanced dementia, our community of over 2000 live-in carers are ready to work. Plus, our sophisticated supply funnel can expand our roster as required. It means we’re hugely adaptable to our partners’ requirements.

Only professionals you can rely on

Most of the professionals we work with have a minimum of one year social care experience, and an average of six. They all undertake a rigorous in-person interview, full background check and must prove specialist experience.
